Facility Evaluation Report
Licensing Program Analyst (LPA) Ramon Serrano conducted an unannounced case management visit-Legal/non-compliance visit. LPA Serrano identified himself and was granted entry by licensee/operator Victoria Legaspi. LPA met with licensee/operator Legaspi and discussed the purpose of today’s visit.
LPA was immediately informed by Licensee that the facility no longer had residents in care. During today’s visit, LPA Serrano was escorted throughout the facility. LPA inspected each room and the outside perimeter and confirmed that their were no residents in care. Licensee stated that she was advised by the resident's case worker that Resident #1 and Resident #2 were transferred to Carroll's Residential Care and Resident #3 remains at the hospital.
No deficiencies were cited or observed on this date.
An exit interview was conducted, and a copy of this report was given to Victoria Legaspi at the conclusion of the visit. The signature below confirms receipt of these documents.
