The inspector’s account

Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced case management visit-Legal/non-compliance visit. LPA Lopez identified herself and was granted entry by licensee/operator Victoria Legaspi. LPA met with licensee/operator Legaspi and discussed the purpose of today’s visit.

During today’s visit, LPA Lopez spoke with licensee/operator Legaspi and resident #1 (R1). LPA verified residents' health and safety, and spoke with licensee/operator Legaspi. LPA confirmed that Ms. Victoria Legaspi was still operating the home as a facility, and three residents still lived at this address.

During the visit, LPA provided R1/tenant with the LIC 986 letter as they were onsite. Per Victoria Legaspi, resident #2 (R2)/tenant was out at their day program and would not return until midafternoon, around 3:30 PM; and resident #3 (R3)/tenant was currently being seen at a local hospital. LPA provided Ms. Legaspi with a hard copy of the LIC 986 for R2 and R3 for their review. LPA also provided Ms. Victoria Legaspi a Notice of Operation in Violation of the Law (NOV) letter informing Victoria Legaspi that they are operating without a license. Prior to LPA's exit, LPA removed the license from the facility.

An exit interview was conducted, and a copy of this report and the NOV letter were provided to operator Victoria Legaspi at the conclusion of the visit. The signature below confirms receipt of these documents.
