Facility Evaluation Report

Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced case management visit-Legal/non-compliance visit. LPA Lopez identified herself and was granted entry by Licensee Victoria Legaspi. LPA met with Licensee Legaspi and discussed the purpose of today’s visit.

During today’s visit, on 01/23/2026, Licensee worked on the notices as described in HSC, Section 1569.38, Posting of licensing reports; disclosure to new resident, and before LPAs exit, LPA confirmed Licensee Legaspi spoke with the residents, their representatives and the LTCO regarding the facility closure due to revocation of facility license and informed them that a written notice is forthcoming.

An exit interview was conducted, a copy of this report, along with Licensee Rights (LIC 9058 03/22) were provided to licensee Victoria Legaspi at the conclusion of the visit. The signature below confirms receipt of these documents.
