Facility Evaluation Report

Licensing Program Analysts (LPAs) Carmen Lopez and Jose De La Cruz made an unannounced visit to conduct a continuation for a Required Annual Inspection that was commenced on 10/20/2025. The facility file was reviewed prior to the visit. LPA was welcomed by, identified themselves to, and discussed the purpose of the visit with licensee Victoria Legaspi.

Due to technical difficulties and time constrains, the annual inspection is being continued on today's date. During today's visit, LPA's Lopez and De La Cruz continued the annual inspection and provided the licensee with the resulted findings.

Deficiencies and civil penalties are being cited during today's visit and may be reviewed on the LIC809-D and LIC421 pages of this report.

An exit interview, and POCs were jointly developed with Licensee Victoria Legaspi to whom a copy of this report along with the Licensee/Appeal Rights (LIC9058 03/22) were provided during today’s visit. Signature below serves as confirmation of receipt of these documents.
