Facility Evaluation Report
Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced case management visit for a Plan of Correction clearance. LPA Lopez identified herself and was allowed entry by Licensee Victoria Legaspi. LPA met with Licensee Legaspi and discussed the purpose of today’s visit.
On 04/02/2025 the facility was issued a civil penalty (CP) due on 04/03/2025. During today’s visit, LPA confirmed that the CP was cleared on 04/09/2025. The CP was not cleared within the allotted time frame and had lapsed 5 days.
New deficiencies were cited during today’s visit and may be found on the LIC809-D page of this report.
An exit interview was conducted and a copy of this report along with Licensee Rights (LIC 9058 03/22) were provided to Licensee Victoria Legaspi at the conclusion of the visit. The signature below confirms receipt of these documents.
