Facility Evaluation Report

During an office visit with the Licensee, Victoria Legaspi, the San Diego Regional Manager (SDRM), Jerry Romero, Licensing Program Manager (LPM), Robyn Clark, and Licensing Program Analyst (LPA), Carmen Lopez, provided this case management report for recurring issues at the facility. SDRM Romero, LPM Clark, and LPA Lopez identified themselves at the commencement of the visit. SDRM, LPM, and LPA met with Licensee Legaspi and discussed the purpose of today’s visit.

During today’s visit at the San Diego Regional Office, SDRM, LPM, and LPA spoke with the Licensee regarding the chronic issues pending at their facility. Deficiencies were cited during today’s visit and may be found on the LIC809-D page of this report.

An exit interview was conducted, a plan of correction was jointly developed, and a copy of this report, along with Licensee Rights (LIC 9058 03/22), were provided to Licensee Legaspi at the conclusion of the visit. The signature below confirms receipt of these documents.
