Facility Evaluation Report

Licensing Program Analysts (LPA’s) Carmen Lopez and Jose De La Cruz conducted an unannounced case management visit-Legal/non-compliance visit. LPA Lopez identified herself and was granted entry by Licensee Victoria Legaspi. LPAs met with Licensee Legaspi and discussed the purpose of today’s visit.

During today’s visit, LPA’s Lopez and De la Cruz toured the facility, requested and obtained relevant documents, spoke with residents and verified their health and safety, and spoke with Licensee Legaspi. LPAs confirmed that Licensee received the documents and understood their contents. LPA’s explained to Licensee Legaspi, the expectations of their daily operations henceforth.

An exit interview was conducted, a copy of this report, along with Licensee Rights (LIC 9058 03/22) were provided to licensee Victoria Legaspi at the conclusion of the visit. The signature below confirms receipt of these documents.
