Facility Evaluation Report

Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced visit for a POC correction and, in conjunction, completed this comprehensive visit initially initiated on April 17, 2025. LPA Lopez identified herself and was granted entry by Victoria Legaspi, Licensee. LPA stated the purpose of the visit and reviewed the basic elements of the visit with Licensee Legaspi.

During today’s visit, LPA completed the Comprehensive Visit Inspection Tool. There were deficiencies observed and cited and may be reviewed on the LIC809-D page of this report.

An exit interview and a POC was jointly developed with Licensee Legaspi during this visit. A copy of this report, LIC 811 and Licensee/Appeal Rights (LIC 9058 3/22) were provided to the Licensee Legaspi at the conclusion of the visit. The signature below confirms the documents were received.
