Facility Evaluation Report
Licensing Program Analyst (LPA) Alyssa Ramirez conducted an unannounced case management visit for a Plan of Correction clearance and in conjunction conducted a health and safety check for clients in care. LPA identified herself and was granted entry by Licensee Victoria Legaspi. LPA met with Licensee Legaspi and discussed the purpose of the visit.
On 10/28/24, the facility was issued six deficiencies regarding updated Administrator’s Certificate, and incomplete staff and client files.
During today’s visit, Licensee informed LPA that deficiencies have not been corrected.Civil penalties are being issued for failure to correct and will be ongoing until corrected.
No new deficiencies were issued today. Civil penalties were issued. An exit interview was conducted and a copy of this report along with Licensee Rights (LIC 9058 03/22) were provided to Licensee whose signature below confirms receipt of these rights.
