Facility Evaluation Report

Licensing Program Analyst (LPA) Arian Golbakhsh conducted a unannounced Case Management visit to collect the license of the facility as the facility underwent a change of ownership and was issued a new one (Bliss Assisted Living & Memory Care #374604986) for the same address. The facility file and personnel report was reviewed prior to the visit. LPA was welcomed by, identified themselves to, and discussed the purpose of the visit to Administrator Megan Amey. During today's visit, there were fifty-six (56) residents in care.

The Centralized Applications Bureau (CAB) had approved the change of ownership effective date 4/13/26. LPA collected the physical license for this facility and ensured that the mass transfer of staff associations from this facility to the newly licensed one was complete.

No immediate health and/or safety concerns were noted during the visit. No deficiencies were cited during the visit either. An exit interview was conducted with Administrator Amey to whom a copy of this report was provided. Their signature below confirms receipt of this document.
