Facility Evaluation Report
Licensing Program Analyst (LPA) Debbie Correia visited the facility to conduct an annual required licensing inspection. LPA Correia was greeted by Front Lobby Receptionist Zaira Acosta , and met Wellness Director Cecy (WD) Salomon, and explained the purpose of the visit.
The facility is Licensed to serve 50 residents ages 60 and above; all of whom may be non-ambulatory. The facility also has a Hospice waiver for 3 residents and the facility's current census is 29.
During today's visit, LPA Correia conducted resident and staff records reviews which were complete and up to date. The facility’s last disaster drill was conducted on February 28, 2025. A review of the facility infection control was up to date and infection control practices are in place. The facility maintenance tests residents' rooms’ water and ambient temperature and maintains a log on file. The facility provides residents with a monthly activity calendar and staff conduct daily reminders for resident engagement. The facility also provides a monthly menu along with a menu of alternative choices for meals.
LPA, accompanied by WD Salomon, conducted a brief tour of the facility. The facility was observed to be clean, sanitary, and in good repair. The fire extinguisher(s)/system was current. Smoke and carbon monoxide detector(s) were present and operational. The facility fire system is hard-wired to the fire department and the facility is equipped with a sprinkler system. There was a large activities room and outdoor shaded area for residents in care. Medications were in locked medication carts and housed in a locked med-tech room. Administration of medications are documented in an electronic MAR system.
Due to time restraints, WD Salomon was notified that the remainder of the annual inspection will be completed at a later date. No deficiencies were cited during today’s initial Annual Inspection. An exit interview was conducted with WD Solomon and a copy of this report, along with the Licensee Rights (LIC9058 FAS 01/16) will be provided at the conclusion of the visit.
