Facility Evaluation Report

Regional Manager (RM) Jerry Romero, Licensing Program Managers (LPM) Robyn Clark and Licensing Program Analyst (LPA) Liliana Silveira conducted an unannounced Case Management - Legal/Non-compliance visit. RM, LPM, and LPA were greeted and allowed entry into the facility by Assistant Manager Marianne Alfaro, to whom they explained the purpose of the visit. Licensee Ligaya Sanders arrived shortly after.

On 06/23/26, the Liicensee was issued a Temporary Suspension Order and clients were relocated. Today, RM, LPM and LPA toured the facility and verified that there were no clients in care. Immediate exclusion orders were also issued to the Licensee as well as to three (3) staff members.

The Department conducted a tour of the home and confirmed that three (3) of the four (4) clients were safely relocated (C1-C3). Client #4 (C4) was assessed by a nearby facility, but declined to leave. LPM conducted a review of C4’s records which revealed that C4 was deemed independent and was able to manage their own medications. RM witnessed Licensee release all medications to C4 and explained to the Licensee that going forward, no elements of care and supervision may be provided, as the facility is no longer licensed.

An exit interview was conducted and a copy of this report, along with Licensee Rights (LIC 9058 03/22) were provided to Ligaya Sanders, whose signature below confirms receipt of the reports.
