Facility Evaluation Report

Licensing Program Analyst (LPA) Liliana Silveira and Licensing Program Manager (LPM) Robyn Clark conducted an unannounced Case Management (CM) visit to to follow-up on an eviction related issue. LPA and LPM were greeted by and discussed the purpose of the visit with Marianne Alfaro, who granted entry to the facility. Licensee Ligaya "Beth" Sanders arrived shortly after.

On 06/18/2026, the Department conducted a visit regarding an unrelated complaint investigation [08-AS-20260616144833]. During the visit, LPA Ngallo conducted interviews regarding one client (C1). Licensee (LIC) indicated that C1 was evicted on 06/11/2026, and that an eviction notice was posted on their door.

On 06/19/2026, LPA De La Cruz visited the facility and conducted further interviews regarding the eviction. Staff #1 (S1) confirmed that C1 was served an eviction. When LPA De La Cruz requested a copy of the eviction, S1 explained that the eviction was provided to C1s case manager, and that they did not have it available as they did not print a copy or retain one for the facility. On 06/23/2026, LPA conducted a record review, which further confirmed that no eviction was received at the San Diego Regional Office as required.

Therefore, a citation was issued in accordance with Title 22 of the California Code of Regulations.

An exit interview was conducted with Marianne and Ligaya, to whom a copy of this report, LIC 809(D) and the Licensee/Appeal Rights (LIC9058 03/22) were provided. Signature confirms receipt.
