Licensing Program Analyst (LPA) Liliana Silveira and Licensing Program Manager

(LPM) Robyn Clark conducted an unannounced Plan Of Correction (POC) visit to deliver amended reports originally issued on 06/20/26. LPA also came to assess required civil penalties. LPA and LPM were greeted by and discussed the purpose of the visit with Assistant Manager Marianne Alfaro, who granted entry to the facility. Licensee Ligaya Sanders arrived shortly after.

On 6/21/2026, the facility received a citation for accessible chemicals. Because this is a repeat citation, a repeat civil penalty is hereby assessed for 6/21/2026 [see LIC 421-FC]. On 6/17/2026, the licensee was issued a repeat-citation for severe pest activity in the facility, for which a 24-hour correction was agreed upon. During the Department’s visit on 6/19/2026, the Licensee was issued civil penalties for failure to correct the violation by the POC due date. During the department’s visit on 6/20/26, severe pest activity was again observed, for which a citation and civil penalty were issued. On 6/21/2026, and 6/23/2026, evidence of continued severe pest activity was still present, based on LPA observations and interviews. The facility exhibited widespread pest infestation, with heavy cockroach activity observed throughout the premises. Live cockroaches were documented on floors, walls, food contact surfaces, kitchen equipment, dry storage shelving and directly on food items. On 06/21/26, LPA Golbakhsh observed a live cockroach crawling on the arm of Staff #1 (S1). On today's date, an inspection was conducted which revealed that pest control provided service on yesterday's date, however, pest activity was still present, which included several cockroaches and four (4) new rat droppings in a drawer near the kitchen sink.
