The inspector’s account

Licensing Program Analyst (LPA) Arian Golbakhsh and Supervising Special Investigator (SSI) John Rante conducted an unannounced Health and Safety visit to the facility. LPA and SSI were welcomed by, identified themselves to, and discussed the purpose of the visit to Staff Thomas Gilbert, Volunteer Lisa Vance, and Assistant Manager Marianne Alfaro.

During today's visit, LPA and SSI toured the facility and inspected each room. One (1) client was present and the other three (3) were out in the community. Assistant Manager Alfaro expressed no new concerns regarding Client #1 (identified as C1) since the Department's previous visit the day before (6/20/26). LPA advised to contact local law enforcement if necessary should C1's behavior escalate.

While touring the kitchen area, LPA noted various chemical items out in the kitchen and living room floor. Per staff, they emptied out the kitchen cabinets/drawers yesterday to clean and treat for pests. LPA explained that chemical items still needed to be stored in secured/inaccessible areas. One Type A citation was cited for the accessible chemicals and are noted to on the attached LIC 809-D. Staff immediately moved the chemical items and locked them in a secured cabinet.

One deficiency was cited during the inspection. An exit interview was conducted with Assistant Manager Alfaro to whom a copy of this report and the Licensee/Appeal Rights (LIC 9058) were provided. Their signature below confirms receipt of these documents.
