Facility Evaluation Report
**This is an amended version of a report originally issued on 6/20/2026** Licensing Program Analyst (LPA) Liliana Silveira, Licensing Program Manager (LPM) Simon Jacobs and Supervising Special Investigator (SSI) John Rante conducted an unannounced Health & Safety inspection. LPA, LPM and SSI introduced themselves and were allowed entrance into the facility by volunteer Lisa Vance. Assistant Manager Marianne Alfaro (S1) was in their room and joined shortly after.
During the visit, LPA, LPM and SSI toured the facility and observed that the main refrigerator in the kitchen was full of food. LPA and LPM inspected the food and found that it was edible and none of the food was expired. The refrigerator temperature was registered at 42 degrees F. LPA inspected food stored in a separate facility freezer and observed that the food was recently purchased and had future expiration dates. A walk through was also conducted through the halls and bedrooms. Staff and other clients did express concerns for their safety due to the behaviors of Client #1 (C1) and were advised to contact local law enforcement if necessary. During the time at the facility, LPA and LPM observed cockroaches crawling on dishes, in the cupboard, crawling at the entrance to the kitchen and several crawling on the living room table while LPA was typing on the laptop. There were no other health & safety concerns observed.
Based on direct observations and interviews, the facility has unsanitary living conditions. On 06/17/26 a repeat-citation was issued for unsanitary living conditions, including pest activity. As of today's date of 06/20/26, the plan of correction has not been met, for which failure to correct civil penalties will be assessed [see corresponding LIC 421-FC, reissued on 6/23/26]. Alfaro was advised that civil penalties will continue until proof of correction is provided. An exit interview was conducted with Alfaro, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided. Signature confirms receipt.
