Facility Evaluation Report
Licensing Program Analyst (LPA) Jose De La Cruz conducted an unannounced Case Management visit to the facility. LPA introduced himself and discussed the purpose of the visit with staff (S1).
During the visit, LPA toured the facility and interviewed staff and clients. It was noted that two incidents occurred the day before in which a client was violent towards staff and a volunteer. LPA requested facility records and inspected the facility's refrigerator and the facility for bugs. The refrigerator was found with a gallon of milk,71 eggs, two loaves of bread, two toppers with food and at 50 degree's Fahrenheit per facility's thermometer. LPA measured temperatures of items inside the fridge on Fahrenheit degrees at 51.4, 50.1 and 49.7. (Licensee arrived at the facility with groceries at around 1:00 PM). On a second measure at 5:00 PM, the fridge temperature was measured at 60 degrees Fahrenheit. The mini-freezer located in the living room was clean and with three bags of frozen goods.
The facility was checked for pests. LPA and S1 opened every door in the kitchen cabinets where cockroaches were observed. Rat traps were observed in different areas of the kitchen, close to the floor next to a kitchen pot containing uncooked rice.
During interviews with staff and clients it was confirmed that the facility is aware of the cockroach and rat problem.
Based on direct observations and interviews, the facility is not cleared from pests. Furthermore, the fridge located in the kitchen that was bought the previous day (06/18/2026) is still not compliant with the required maximum 45 Fahrenheit degrees of temperature.
