Facility Evaluation Report
Licensing Program Analyst (LPA) Janet Ngallo conducted an unannounced Case Management visit. LPA introduced themselves and discussed the purpose of the visit with Licensee Ligaya Sanders.
During the visit, LPA toured the facility and observed that the main refrigerator in the kitchen was empty and out of temperature. LPA and the Licensee inspected food stored in a separate facility freezer and inside a mini-refrigerator located in a staff room. The mini-refrigerator contained visible dirt and roaches. All perishable food items reviewed that were stored in the freezer and the resident’s mini-refrigerator were found to be expired at the time of inspection.
Interviews revealed that clients and staff were aware of expired food in the facility and therefore limited the amount of food consumed at the facility to non-perishable foods or foods purchases outside of the facility. Interviews also confirmed that perishable foods served at the facility had, at times, been expired. Staff(S1) reported serving chicken adobo for dinner the previous day and cereal and bread with jelly earlier that day. S1 acknowledged that the food stored in the refrigerator was spoiled and confirmed that the food used for meal preparation had been purchased elsewhere because the stored food was not safe to use.
Based on direct observations and interviews, the facility did not maintain perishable food in good condition and safe to consume, and clients were exposed to expired food products.
Licensee was additionally reminded that the Plan of Correction for deficiencies cited on 06/17/2026 for pests and inoperable fridge was due today, and that failure to correct will result in commencement of civil penalties.
