Facility Evaluation Report

Licensing Program Analysts (LPAs) Eryn Kane and Amy Domingo conducted an unannounced Case Management visit. LPAs introduced themselves and stated the purpose of the visit with Assistant Manager Marianne Alfaro.

During visit LPAs toured the facility and observed live cockroaches in the kitchen. Additionally, there were both live and dead insects, including visible insect droppings, discovered in the refrigerator.

These observations result in a repeat violation and incur a Civil Penalty (refer to the attached LIC 9099-D).

A Plan of Correction was jointly developed with the licensee. An exit interview was conducted with House Manager Marianne Alfaro, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were delivered electronically.
