Facility Evaluation Report

Licensing Program Analyst (LPA) Ramon Serrano conducted an unannounced Case Management visit. LPA met with Assistant Manager (AM) Marianne Alfaro and we discussed the purpose of the visit.

During today's unannounced complaint visit AM advised LPA that Client 1 (C1) passed away. AM stated that she was advised by a Telecare Case Worker on 4/28/26 that C1 had passed away on 4/26/26. AM stated that she did not have anymore details and referred LPA to Telecare for more information. LPA spoke with a Telecare Team Leader and gathered information regarding the death of C1.

No deficiencies were cited or observed on this date.

An exit interview was conducted with Marianne Alfaro who was provided with a copy of this report and Appeal Rights. Their signature confirms receipt of these documents.
