Facility Evaluation Report
Licensing Program Analyst (LPA) Marisela Garcia-Centeno, conducted a Case Management Visit during a complaint visit. LPA met with Caregiver, Alma Bayau and we discussed the purpose of the visit. LPA discussed with Administrator Jofel Velasco via telephone the procedures discussed with staff during the viist.
During today's visit LPA discussed with staff entrance procedures to the facility through the main front door entry. Staff were directing visitors to enter through the back due to procedures that had been put in place during COVID-19 infection control.
No deficiencies were cited or observed on this date.
An exit interview was conducted with Caregiver, Alma Bayau and provided with a copy of their appeal rights (LIC9058 03/22) along with a copy of this report.
