Facility Evaluation Report
On 4/25/25, Licensing Program Analyst (LPA) Arian Golbakhsh conducted a Licensee initiated Case Management visit. Licensee had wanted guidance from the Community Care Licensing Department regarding the retention of a resident who no longer fit the age-limit range for the facility's license, C1. LPA was welcomed by, identified themselves to, and discussed the purpose of the visit to Assistant Administrator Edgar "Jerry" Dacanay.
LPA conducted file review of C1's files and provided consultation with Assistant Administrator Dacanay of the regulations for retaining aging residents and documentation needed. LPA was unable to conduct an interview with C1 as they were out in the community.
No deficiencies were cited during today's visit. An exit interview was conducted with Assistant Administrator Dacanay to whom a copy of this report was provided. Their signature below confirms receipt of this document.
