Licensing Program Analyst (LPA) Debbie Correia conducted an unannounced Case Management visit. LPA was welcomed by and identified herself to Marketing Regional Michael Stickler. LPA then met and discussed the purpose of the visit with Executive Director (ED) Adrian Guillen.
LPA conducted a visit to obtain signatures on amended complaints.
An exit interview was conducted with ED Guillen, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 01/16) were provided.
