Facility Evaluation Report

Licensing Program Analyst (LPA) Dang Nguyen conducted an unannounced Case Management visit. LPA was welcomed by and identified himself to Concierge Andrea Maldonado-Odgers. LPA then met and discussed the purpose of the visit with Sales Director Jackie Comardo.

LPA conducted a brief tour of the facility and observed staff and residents in care. During today's visit, LPA provided technical assistance / education on how to update the name of the facility administrator on record with CCLD. No deficiencies were cited on this date.

An exit interview was conducted with Comardo, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 01/16) were provided.
