The inspector’s account

Licensing Program Analyst (LPA) Amy Rodgers conducted an unannounced Case Management - Incident visit. LPA identified herself to, was greeted by, and explained the purpose of the visit to Administrator Kristen Hollobaugh. Prior to the visit LPA Rodgers met with Director of Consumer Services Diana Spurgeo at DSC- Morton house.

During an Annual inspection of the DSC- Morton House LPA A. Rodgers was informed by Diana Spurgeo that Client #1 (C1) experienced an incident while attending the day program at the LInk Center.

C1 is non verbal and blind. According to interview with the Administrator, C1 uses their hands as a guide to maneuver around a room. C1's finger became caught in door as it was closing. C1 was taken to the emergency room and was given 3 stitches and a PRN. LPA was able to observe C1 while at DSC- Morton house. C1 was alert, well groomed and did not seemed bothered by pain. LPA was unable to conduct an interview with C1 due to their physical and cognitive abilities. No Deficiencies were cited during the visit as the facility responded accordingly and appropriately.

An exit interview was conducted with Administrator Kristen Hollobaugh with whom a copy of this report was provided. Their signature below confirms receipt of this document.
