Facility Evaluation Report

Licensing Program Analyst (LPA) Amy Rodgers conducted an unannounced visit to commence a annual inspection. he facility file was reviewed prior to the visit. LPA identified herself and discussed the allegations mentioned above with Laika Aguinaldo Administrator .

During today’s visit, LPA toured the facility property. LPA also reviewed resident files. Due to time constraints, the inspection will be continued on a separate date.

An exit interview was conducted and a copy of this report along with Licensee Rights (LIC 9058 03/22) were provided to Laika Aguinaldo Administrator whose signature below confirms receipt of these rights.
