Facility Evaluation Report

Licensing Program Analyst (LPA) Amy Rodgers conducted an unannounced case management visit to a request to increase the facility's capacity of the facility. LPA was greeted entry by Patricia Barber, identified herself, and discussed the purpose of the visit with Patricia Barber.

A Change of Capacity from 3(three) to 4(four) ambulatory clients application was received by the Department on April 7, 2025, in which the licensee requested in an increase of 1 clients (from 3 to 4).n The Fire Safety Inspection Request was approved by the local fire authority on April 21, 2025.

During today’s visit, LPA conducted a facility tour.The completed change of capacity will be forwarded to management for final review and approval. An exit interview was conducted with Patricia Barber, to whom a copy of this report and the Licensee Rights (LIC9058 01/16) were provided at the end of the visit.
