Facility Evaluation Report

Licensing Program Analyst (LPA) Angelica Boyles arrived at the facility at 8:15 am to conduct an unannounced annual inspection. LPA knocked on the facility door several times and waiting for an answer. LPA then called the facility number 619-439-7791, but there was no answer. LPA then called the facility's mobile number, and there was no answer. LPA left a voicemail with a callback number at both phone numbers. LPA attempted to contact both numbers several more times with no answer. LPA waited for over 30 minutes at the facility, but nobody showed up. LPA Left the facility at 9:04 am. LPA will return at a later date to conduct the annual required licensing inspection.
