This is an official report of an unannounced visit/investigation of a complaint received in our office on
11/08/2022 and conducted by Evaluator Tiffany Holmes
COMPLAINT CONTROL NUMBER: 08-AS-20221108145856
Client was isolated causing a personal rights violation.
Licensing Program Analyst (LPA)Tiffany Holmes contacted the facility to deliver findings for a complaint investigation via tele-virtual. LPA identified herself to, and explained the purpose of the visit and the basic elements of the complaint with Blanca Vazquez Program Administrator.
The Department’s investigation consisted of unannounced facility visits, interviews with facility staff, residents, and outside sources. It was alleged that the client was isolated causing a personal rights violation. Interviews with staff revealed what they can recall from 2022 is that they were all isolated because covid was still going on. Interviews revealed that staff and clients still were getting covid and if the clients came to program and tested positive for covid they would remove the clients from where all the other clients were and have an isolation room for them to go into with staff. Interviews revealed that staff were trained to put the gowns on along with facemasks to work with the clients that were covid positive. Interviews revealed that the clients would go in the room until they got picked up from their responsible parties. Interviews revealed that the staff did not leave the clients unattended or isolated the clients. Interviews revealed that the staff deny isolating any of the clients.
The Department has investigated the above allegation. Based upon the available information obtained during this investigation the allegation may have happened or is valid, but there is not a preponderance of evidence to prove that the alleged violation occurred. The allegation is unsubstantiated.
An exit interview was conducted with Blanca Vazquez Program Administrator to whom a copy of this report and the Licensee’s Rights (LIC9058 01/16) were provided.
