The inspector’s account

Licensing Program Analyst (LPA) Angelica Boyles conducted an unannounced case management visit. LPA was greeted and allowed entry into the facility by Bao Tu, owner and discussed the purpose of the visit regarding a decrease in capacity.

The Licensee applied for a decrease in capacity from six (6) clients to four (4) clients. There is an approved Fire Clearance dated 10/20/2025 for a total capacity of four (4) clients in care, two (2) may be ambulatory and two (2) may be non-ambulatory. LPA toured the facility and observed it to reflect the updated Fire Clearance and floor plan.

No deficiencies were observed or cited during today's today.

An exit interview was conducted with Bao Tu, owner. A copy of this report and the Licensee Rights (LIC 9058 03/22) were provided and signature on this form acknowledges receipt.
