Facility Evaluation Report
Licensing Program Analyst (LPA) Angelica Boyles made an unannounced visit to the facility to conduct the required annual licensing inspection. LPA was met by House Manager Rae Holmes, identified herself, was granted entry into the facility, and stated the purpose of today’s visit. The facility is licensed to serve 4 clients, ages 18 to 59, all of whom must be ambulatory. LPA, accompanied by House Manager, conducted a facility tour. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. The facility's ambient internal temperature was comfortable and complaint and hot water at taps accessible to clients measured at temperatures within compliant range. Disinfectants, cleaning solutions, poisons, and sharp objects were locked and inaccessible to clients. Client rooms were equipped with the required furnishings. Extra linens and hygiene supplies were present. LPA observed smoke alarms, and carbon monoxide detectors throughout the facility that were in operable condition. First aid kit was complete and readily accessible. Per House Manager, there are no weapons and/or ammunition housed in the facility. No pools or bodies of water were observed on the premises. Required licensing postings were observed in visible areas of the facility.
The facility is stocked with at least a 2-day supply of perishable and 7-day supply of nonperishable food items. The food was observed properly stored. Medications are stored in a locked cabinet and administered according to the label instructions. The facility does not manage cash resources or P&I for the clients in care. LPA reviewed client and staff records. Records reviewed were up to date and contained the required documentation. Administrator certification was also current.
Based on today's visit, there were no deficiencies observed or cited. An exit interview was conducted and a copy of this report will be provided to House Manager Rae Holmes. Signature confirms receipt of the documents.
