The inspector’s account

Facility staff heard the alarm to the facility’s back door and went towards the front door to observe C1 was exiting the facility property via the driveway gate. Staff 1 (S1) followed C1 down the street and attempted to redirect C1 while Staff 2 (S2) remained on facility property and contacted law enforcement. Interviews and review of incident reports revealed that S1 was not able to redirect C1 to return to the facility and C1 engaged in property destruction while at the neighboring property. Law enforcement was summoned by facility staff to assist. Interviews provided conflicting information regarding the details of C1’s behavior and actions prior to C1’s elopement however, the information provided during interviews was consistent in that C1 was not unsupervised for any period of time after C1 was observed by the front driveway gate. Interviews and incident reports reviewed revealed that C1 was transported to the hospital and returned to the facility the next day with no noted injuries.

The Department has investigated the above-mentioned allegation and based on interviews and records review, the preponderance of the evidence has not been met, therefore, this allegation is deemed unsubstantiated.
