Facility Evaluation Report
Licensing Program Analyst (LPA) Angelica Boyles conducted an unannounced Case Management - Incident visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Administrator Silvia Davalos.
Today's visit was in response to an Incident Report, which Licensee submitted to the CCLD San Diego Regional Office (received on 10/21/25). According to the report: on 10/20/25 it was revealed that Staff #1 (S1) had accessed electronic client records for Client #1 (C1) . [See LIC811 Confidential Name List for a description of select person identifiers used in this report.] The report states S1 violated the facility company policy by "accessing and copying confidential client information without authorization". Per Administrator, as of today S1's access to the online electronic records has been revoked.
During today’s visit, LPA performed a facility tour, welfare check, collected records, and interviewed staff.
A deficiency is being cited per California Code of Regulations, Title 22 (refer to the attached LIC 809-D)
An exit interview was conducted with Administrator Silvia Davalos, to whom a copy of this report, the LIC811 Confidential Names List, and the Licensee/Appeal Rights (LIC9058 03/22) were provided during the visit.
