Facility Evaluation Report
Licensing Program Analyst (LPA) David Roman conducted an unannounced Case Management Inspection. LPA D. Roman identified himself to Cristina Salazar, and discussed the purpose of the visit.
LPA D. Roman reviewed the incident report that was sent to CCLD SDRO of the incidents that occurred on 06/08/26. LPA D. Roman also reviewed staff notice of termination of employment. LPA D. Roman conducted staff interviews which revealed the staff and Administrator had walked the ex-staff out of the facility.
LPA D. Roman along with facility staff toured the interior and exterior of the facility and inspected bedrooms and bathrooms. The facility was sanitary, and in good repair. Interior pathway has an unused washer which may cause an obstruction, however one can walk by and the facility was free of slip hazards. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and resident activities.
Cooking/dining equipment and utensils were present. There were no toxic chemicals/poisons accessible to residents, medication cabinet is locked. No pools or bodies of water on the premises.
An exit interview was conducted with Facility Admin, Cristina Salazar.
