Based on interviews and a records review, the above allegation was is determined to be unsubstantiated. A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
An exit interview was conducted with Assistant Director Cave whom was provided a copy of this report and Licensee Appeal Rights (LIC 9058) have been given for facility records. Signature below confirms receipt of the documents.
