Facility Evaluation Report
LPA D. Roman conducted an unannounced case management inspection while investigating an unrelated matter. LPA D. Roman introduced himself, identified himself, and discussed the purpose of the visit with Caregiver, Julita Renojo.
LPA along with facility staff conducted a tour of the interior and exterior of the facility. LPA D. Roman inspected 5 bedrooms and 3 bathrooms. While touring the interior of the facility 1 bedroom door was missing a door knob. While conducting an exterior tour of the facility, LPA D. Roman observed junked hazard materials (old bed, rusted metal bed springs, old tree branches, spare wood parts, junked lawn equipment, wooden fence pieces, as well as wooden table legs parts and rusted metal chair parts). LPA D. Roman informed the Caregiver that the junk hazards need to be removed and that the door knob needs to be replaced as they pose a potential health and safety risk to residents in care.
An exit interview was conducted with Caregiver, Julita Renojo to whom a copy of this report, deficiencies page, and Licensee/Appeals Rights (LIC 9058) were provided.
