Facility Evaluation Report

Regional Manager (RM) Jerry Romero, Licensing Program Manager (LPM) Simon Jacob, and Licensing Program Analyst (LPA) Tiffany Holmes met with Licensees Samsam Khalif, Ramla Sahid and Abdiaziz Hussein in the office and discussed the purpose of the meeting.

On 12/03/2025 the Licensee was informed that there was an order to individual of exclusion to operate Resource Family Homes and Department Licensed Facilities. This meeting discussed the plan of action and non immediate exclusion of Licensee Samsam Ahmed Khalif. Licensee Samsam Ahmed Khalif stated that they sent the appeal on 12/10/2025. If the appeal is denied, the Licensee's plan is to buyout the current partner or get another partner. Licensee Samsam Ahmed Khalif does not reside at the facility nor does she provide any care or supervision to any of the residents. There are no further issues during the meeting.

An exit interview was conducted with Ramla Sahid Licensee. A copy of this report and Licensee's Rights (LIC 9058 03/22) were provided to the Licensee and their signature on this report confirms receipt of the Licensee's Rights.
