Lack of supervision resulting in client altercation

Licensing Program Analyst (LPA)Tiffany Holmes contacted the facility to deliver findings for a complaint investigation via tele-virtual. LPA identified herself to, and explained the purpose of the visit and the basic elements of the complaint with Alexis "Lexie" Garcia, Program Director.

Licensing Program Analyst (LPA) Domingo previously conducted a tour, made observations, and obtained and reviewed pertinent records on 04/10/2025. LPA Holmes conducted interviews with clients, staff, and outside sources.

It was alleged that lack of supervision resulting in client altercation. Interviews revealed that the clients are supervised while at program. Interviews with staff revealed that Client one (C1) moved really fast and started to strangle C2. Staff intervened quickly and separated clients by redirecting them. Interviews revealed there were two staff at program supervising the clients. Interviews revealed there were no injuries and an incident report and SOC341 was filled out and submitted to Licensing and San Diego Regional Center (SDRC). Interviews revealed that after this incident and due to C1's aggression, they applied for 1:1 supplemental support for C1 through the SDRC. Interviews revealed the supplemental support was approved in August 2025. Interviews with clients revealed that C1 was unable to be interviewed due to being non verbal and C2 has passed away. The Department has investigated the above-mentioned allegation and based on interviews, LPA observations, and records review, it was determined that the complaint allegation is unsubstantiated. The allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.

An exit interview was conducted with Alexis "Lexie" Garcia, Program Director via face time and a copy of this report along with Licensee/Appeal Rights (LIC 9058 03/22) was provided via email. An electronic email read receipt confirms the documents were received.
