Facility Evaluation Report
Licensing Program Analyst (LPAs) Amy Rodgers and Janet Ngallo conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPAs were welcomed by, identified themselves to, and discussed the purpose of the visit with Administrator Lexi Garcia.
According to the facility’s license, the facility has a maximum capacity of 60 (60) clients, all of whom must be ambulatory. During today’s inspection, there were a total of 52 (52) clients in care. This facility does not feature a secured perimeter or delayed egress doors.
LPA's accompanied by licensee’s staff, toured the interior and exterior of the facility, and inspected each area. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. A room is provided for rest periods. Doors, windows and screens, and toilets were in working order. The facility’s ambient internal temperature was compliant. Hot water temperature at taps accessible to clients were all compliant. The program does not supply lunches, However can supply snacks and lunches if participants did not bring a lunch. In addition, LPA's observed a separate area to purchase low cost snacks.
Due to time restraints, Program Director, Alexis Garcia was notified that the remainder of the annual inspection will be completed at a later date.
No deficiencies were cited during today’s initial Annual Inspection.
An exit interview was conducted with Program Director, Alexis Garcia and a copy of this report will be provided at the conclusion of the visit. The signature below confirms receipt of the reports.
