Licensing Program Analyst (LPA) Arian Golbakhsh conducted an unannounced Case Management visit to offer an Amended Report for a visit conducted on 6/23/2025. LPA met with Associate Executive Director Aileen Spence and informed them of the purpose of their visit.
During today's visit, LPA obtained Associate Executive Director Spence's signature on the amended report LIC 9099D (7/18/2025).
An exit interview was conducted with Associate Executive Director Spence to whom a copy of this report and the Licensee/Appeal Rights (LIC 9058) were provided. Their signature below confirms receipt of these documents.
