Licensing Program Analysts (LPA) Amy Domingo conducted an unannounced collateral visit as a follow-up for an unrelated complaint investigation for another facility. LPAs were welcomed by, identified themselves to, and discussed the purpose of the visit to Executive Director .
During the visit, LPA reviewed document with LPA Golbakhsh regarding a few clients.
An exit interview was conducted with Executive Director Cathy Allen to whom a copy of this report was provided. Their signature below confirms receipt of this document.
