This is an official report of an unannounced visit/investigation of a complaint received in our office on

09/25/2025 and conducted by Evaluator Amy Rodgers

COMPLAINT CONTROL NUMBER: 08-AS-20250925095127

Staff are not following proper food handling techniques

Licensing Program Analyst (LPA's) A.Rodgers and R.Hashemi conducted an unannounced visit to investigate and deliver findings regarding the above complaint allegation. LPAs introduced themselves and disclosed the purpose of the visit to Executive Director Cathy Allen. The visit included a tour of the memory care building, including the dining and common rooms, interviews with staff and residents, and a review of facility procedures.

On September 25, 2025, it was alleged that food was left out in the dining room and that residents had previously eaten unattended or dropped food. Department interviews, as well as several past and present observations of the dining area, were observed to be clean and free of unattended food. Staff interviews confirmed that food is cleared promptly and that staff monitor the area after meals. Department interviews with several outside sources revealed that staff are following infection control guidelines. Based on interviews, direct Department observations, and records review, a preponderance of evidence does not exist to prove that staff failed to follow proper food handling procedures; therefore, the allegation is UNSUBSTANTIATED.

An exit interview was conducted with ED Allen, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided.
