The inspector’s account

Interviews with staff and outside sources all corroborated that during the time frame of the received complaint allegation, no malodors were noticed or reported in or around R1's unit. Additionally, during the initial complaint visit conducted by the Department, a moisture analysis walk-through was completed which yielded results indicating that there was no mold present in the areas tested. File review of maintenance history revealed no orders concerning mold/mildew at R1's unit.

Based on interviews and records review, while the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violation occurred – therefore the allegation has been determined to be UNSUBSTANTIATED. An exit interview was conducted with Health Services Director Mendez to whom a copy of this report and the Licensee/Appeal Rights (LIC 9058) were provided. Their signature below confirms receipt of these documents.
