Facility Evaluation Report

Licensing Program Analyst (LPA) Amy Rodgers conducted an unannounced Case Management Visit to observe the physical plant. LPA was welcomed by, identified herself to, and and met with House Manager Patty Alonzo and consultant Henri Chazaud to whom she disclosed the purpose of the visit.

On 9/27/2024, the Licensee submitted an LIC200 Application to the CCLD San Diego Regional Office (RO) requesting to add and Accessory Dwelling Unit (ADU), this will not increase their capacity. The facility’s floor plan changed in that the facility now includes a two (2) bedroom ADU, meant to accommodate two (2) to four(4) non-ambulatory residents.

On 7/30/2025 , the local fire authority approved/granted an updated fire clearance, reflecting the facility was approved six (6) non-ambulatory residents of which one (1) may be bed-ridden.

During today’s visit, LPA briefly toured the interior and exterior of the facility. The facility sketch/floor plan was consistent with the current layout of the facility. No deficiencies were cited for this increase capacity visit.

This portion of the application process has been completed. The Licensee will be sent an updated license to reflect the new fire clearance after CCLD management’s final review and approval.

An exit interview was conducted, and this report was discussed with House Manger Alonzo and Consultant Chazaud and a copy of the report and Licensee Appeal Rights will be provided after the conclusion of the visit. House Manger Alonzo signature on this form acknowledges receipt of reports.
