Facility Evaluation Report
Licensing Program Analyst (LPA) Amy Rodgers, made an unannounced visit to conduct the required One-Year Inspection. LPA Rodgers were granted entry into the facility by Patty Alonzo, House Manager. LPA identified herself and stating the purpose of the inspection. The facility serves 6 elderly residents, age 60 and above, all whom may be non-ambulatory, of which one (1) may be bedridden.
During today’s visit, LPA spoke briefly to staff and residents. Due to time constraints, a return visit on a subsequent day is needed to complete the annual inspection.
No deficiencies were cited during today’s visit.
An exit interview was conducted with Care Patty Alonzo and a copy of this report was provided.
