Licensing Program Analyst (LPA) Rodgers conducted an unannounced visit to further investigate and deliver findings regarding the above complaint allegation. LPA introduced herself and disclosed the purpose of the visit to Lindsay Taylor, Director.

On January 22, 2026, the Community Care Licensing Division (CCLD) received an online complaint alleging that the facility abandoned a resident at the hospital. More specifically, the reporting party claimed that the facility discharged Client 1 (C1) without notice and left C1 at the hospital without assistance or a transition plan.

The Department’s investigation consisted of a review of facility records and interviews with facility staff. Documentation and interviews revealed that C1 had previously been a client at TC1. Records show that the transfer from the hospital back to TC1 was a coordinated effort involving C1, the facility, and TC1. The facility paid for C1’s transportation from the hospital to TC1. Step-by-step communication between the facility, C1, and TC1 was documented. The investigation did not produce supporting evidence or witness statements to substantiate the allegation of abandonment. The allegation that the facility abandoned a resident at the hospital is UNSUBSTANTIATED.

An exit interview was conducted with Lindsay Taylor, Director, to whom a copy of this report, and the Applicant/Licensee Rights (LIC9058 03/22) were provided at the conclusion of the visit.
