R1 would not provide the location. No bite marks were observed or reported by staff. No corroborating evidence was obtained to support that a dog bite occurred while R1 was under staff supervision.
Outside Source 1 (OS1) and Outside Source 2 (OS2) stated that R1 has made similar statements in the past at other facilities and has previously threatened to get facilities shut down when upset.
Regarding laundry services, the Administrator reported that staff assist residents as needed. Staff add detergent, start the washing machine, and transfer clothing to the dryer if residents are away from the facility. Clean clothing is placed in residents’ rooms. R1 is independent with activities of daily living. No documentation or witness statements supported that laundry needs were unmet.
Regarding meals, the Administrator reported that the facility provides daily main course meals and offers alternative options if a resident does not want the meal served. Residents may also order takeout, and R1 participated when takeout was ordered. R1 previously reported being provided smaller portions following a hospital discharge for appendectomy; however, staff indicated this was to prevent vomiting after surgery and that food was not withheld. Residents and staff denied that R1 was denied meals.
Based on interviews conducted and information obtained, there is insufficient evidence to support that staff failed to prevent a dog bite, neglected laundry needs, or failed to ensure R1 was fed. Therefore, the allegations are unsubstantiated. An unsubstantiated finding means that although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred.
An exit interview was conducted and a copy of this report along with the Licensee Rights (LIC 9058) was provided to Administrator. Her signature on this form confirms receipt of the documents.
