Complaint Investigation Report
A review of outside source documentation revealed that the pest control inspections occur every two weeks, and any areas of concern are rechecked for new activity. LPA observations during a tour of the rooms showed that resident rooms had no signs of infestation.
Based upon the information obtained during this investigation, the allegation is unsubstantiated. The allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred.
An exit interview was conducted with Chris Neale, Executive Director, to whom a copy of this report and the Licensee’s Rights (LIC9058 03/22) were provided.
