Licensing Program Analyst (LPA) Amy Domingo conducted an unannounced Collateral visit. LPA was greeted by, identified herself to, and discussed the purpose of the visit with Natalie Carlborg, Executive Director.

During today's visit, LPA observed residents during mealtime/Activities, reviewed residents files and typed interviews.

There were no deficiencies observed during this visit.

An exit interview was conducted with Natalie Carlborg, Executive Director, to whom a copy of this report and the Licensee Appeal Rights (LIC9058 03/22) were provided at the end of the visit.
