Licensing Program Analyst (LPA) Amy Rodgers conducted an unannounced Case Management Visit. LPA was greeted by and met with Resident Care Coordinator, Lima Taiti to discuss the purpose of the visit.

Today's visit is in response to a complaint that was unrelated to the facility. LPA briefly interviewed staff and conducted a brief records review.

An exit interview was conducted with Resident Care Coordinator, Lima Taiti, who was provided with a copy of this report. Their signature confirms receipt of these documents.
